Parental & Student Consent Form - Updated August 2026
Helping us keep your child safe while ensuring we understand and respect your preferences.
San-Marie productions and performances may be filmed and photographed. As students perform as part of a group, it is not possible to exclude an individual student from photographs or recordings of a production.
Performance photographs and recordings may be retained as part of San-Marie Stage School's archive and may be used in the future to celebrate the school's history, previous productions and the development and achievements of our students.
I understand that consent to Performance Media is required for a student to take part in San-Marie productions and performances.
PROMOTIONAL AND SOCIAL MEDIA
From time to time, San-Marie Stage School may take photographs or short video clips during classes, rehearsals, events and school activities.
With your permission, this media may be used to celebrate and promote San-Marie Stage School, including on our website, social media accounts and other promotional or advertising materials.
Where appropriate, media may continue to be used after a student has left San-Marie if it was created and published while valid permission was in place. We will not use a student's full name alongside an image without separate permission.
I give permission for photographs and/or video footage of the student to be used by San-Marie Stage School for promotional and social media purposes.
From time to time, San-Marie teachers may record short video clips of dances, choreography or class work as a teaching aid. This allows teachers and students to review choreography, technique and progress during the learning process. Clips may be shared on the dance groups WhatsApp to allow students to practice at home
These recordings are for teaching purposes only and will not be shared on San-Marie's website or social media under this permission. Recordings will be securely deleted when they are no longer required for the teaching purpose for which they were made.
I give permission for the student to appear in photographs or video recordings taken by San-Marie staff for teaching purposes as described above.
From time to time, photographs or video recordings of class work, dances or choreography may be shared with members of the relevant San-Marie class or group via WhatsApp to support home practice and learning.
These recordings are provided for personal practice purposes only and may include other San-Marie students.
By agreeing to this policy, you understand that any photographs or video footage shared by San-Marie must not be copied, forwarded, uploaded, published or shared on social media or with anyone outside the intended San-Marie class or group.
I give permission for the student to appear in teaching media that may be shared privately with members of their San-Marie class or group for home practice, and I agree not to share or distribute this material outside that group.
To help us ensure the safety and well-being of all students, please read the following information carefully.
Arriving at San-Marie
The studios are accessible 15 minutes before the first class of the day. Please do not enter the building before this time.
San-Marie staff assume responsibility for students during their scheduled classes. Parents/guardians remain responsible for students before their first class, during any breaks between classes and once their final class has finished, except where specifically stated below.
Pre-School & Little Stars
All children who have not yet started school must be accompanied by a parent or responsible adult while on the premises.
Students attending Little Stars may have one accompanying adult with them in the studio.
Reception to Year 6
Students from Reception to Year 6 must be accompanied onto the premises by an adult.
Where a student has consecutive San-Marie classes, staff will escort them from one class to the next.
If there is a break between classes, responsibility returns to the parent/guardian and an adult must be available to supervise the student.
At the end of their final class, students up to and including Year 6 must be collected from the reception area by an adult.
​Year 7 & Above
Students in Year 7 and above may arrive independently and are expected to attend their classes at the correct time.
If waiting in reception before a class, students should wait quietly and responsibly.
At the end of their timetabled classes, students in Year 7 and above may leave the studio independently.
For safety, we recommend that students waiting to be collected remain within the black-railed area outside the studio, with parents driving around the perimeter of the business centre to collect them from the gate.
Leaving San-Marie Early
Once a student has begun their scheduled classes, they must not leave before the end of their timetable unless San-Marie has received permission from their parent/guardian.
Any change to a student's normal collection or leaving arrangements should be reported through the WhatsApp Absentees & Authorisation for Changes List.
If you are unable to access the WhatsApp group, written authorisation should be handed to the teacher at the beginning of the student's class.
Please do not use email for same-day changes to collection or leaving arrangements, as the message may not reach the relevant teacher in time.

Parent/Guardian Agreement
I confirm that I have read and understood San-Marie Stage School's Care & Responsibility arrangements, including when San-Marie staff supervision begins and ends, and I agree to follow the procedures outlined above.
PHYSICAL ACTIVITY, TEACHER CONTACT & PERSONAL CARE
Dance, acrobatic and gymnastic-based activities are physical disciplines which may include stretching, flexibility work, strengthening, balances, partner work, jumps, turns and the learning of new skills.
Students may occasionally experience normal muscle soreness, discomfort or minor strains when developing flexibility, strength or learning unfamiliar movements. As with all physical activity, there is also an inherent risk of accidental injury.
San-Marie teachers will teach skills appropriate to a student's age and ability and will take reasonable steps to provide a safe teaching environment. Students are expected to follow teachers' instructions, work within their own capabilities and inform a teacher immediately if they experience pain, feel unwell or do not feel comfortable attempting an exercise or skill.
Parents/guardians must inform San-Marie of any injury, medical condition or other circumstance that could affect a student's ability to participate safely and must keep this information up to date.
San-Marie cannot accept responsibility for injury arising solely from the inherent risks of participating in dance or acrobatic activity, or where a student disregards safety instructions or undertakes movements outside the activity being instructed, except where San-Marie may be legally responsible.
Appropriate Physical Contact -
Dance and acrobatic teaching may require appropriate physical contact between a teacher and student.
This may include supporting a student while learning an acrobatic skill, assisting with balance or positioning, and using appropriate hands-on corrections to help with posture, alignment, technique, flexibility and safe execution of movements.
Any physical contact will be for legitimate teaching or safety purposes and should be appropriate to the student's age and the activity being taught.
Students are encouraged to tell their teacher if they are uncomfortable with physical contact or would prefer a correction to be demonstrated verbally where this can safely be accommodated.
Toileting & Personal Care -
Students should be able to use the toilet independently unless alternative arrangements have been agreed with San-Marie in advance.
San-Marie teaching staff do not routinely provide intimate or personal care.
Parents/guardians of younger students who may require assistance with toileting or changing should remain available to provide this assistance.
Parent/Student Agreement
I confirm that I have read and understood the information above regarding physical activity, the inherent risk of injury, appropriate teacher contact and personal care arrangements.
I agree that the student will follow teachers' safety instructions and that I will inform San-Marie of any relevant medical condition, injury or change that may affect their safe participation.
FIRST AID & EMERGENCY MEDICAL ASSISTANCE
San-Marie Stage School will take reasonable steps to provide a safe environment for all students. However, accidents, injuries or sudden illness can occasionally occur.
Where necessary, appropriately trained San-Marie staff may provide basic first aid to a student while they are attending the school.
If a student becomes unwell or is injured and requires further attention, San-Marie will make reasonable efforts to contact their parent/guardian or emergency contact using the details provided.
In a medical emergency, San-Marie staff may contact the emergency services and provide them with relevant information held by the school where this is necessary to assist with the student's care.
Where urgent medical assistance is required, the student's safety and immediate medical needs will take priority if a parent/guardian cannot be contacted straight away.
Parents/guardians are responsible for ensuring that the emergency contact and relevant medical information provided to San-Marie is accurate and kept up to date.
Parent/Student Agreement
I confirm that I have read and understood San-Marie Stage School's First Aid & Emergency Medical Assistance arrangements and give permission for appropriate first aid to be administered when reasonably required.
I understand that, in an emergency, San-Marie may contact the emergency services and share relevant information necessary to assist with the student's care.
San-Marie Stage School is committed to providing a safe, positive and supportive environment for all students and staff.
Students are expected to behave respectfully towards teachers, staff and fellow students, follow reasonable instructions and behave in a way that allows everyone to learn and participate safely.
Bullying, intimidation, aggressive or discriminatory behaviour, deliberate damage to property, persistent disruption or behaviour that places the student or others at risk will not be accepted.
Where concerns arise, San-Marie will normally work with the student and their parent/guardian to resolve the situation. Depending on the seriousness or frequency of the behaviour, a student may be asked to sit out of an activity or class and a parent/guardian may be contacted.
In cases of serious or persistent unacceptable behaviour, San-Marie reserves the right to suspend or withdraw a student's place where this is considered necessary to protect the safety, wellbeing or learning environment of students and staff.
Parent/Student Agreement
I confirm that I have read and understood San-Marie Stage School's Behaviour & Conduct expectations and agree that the student will follow these expectations while attending San-Marie.
UNIFORM & PERSONAL BELONGINGS
San-Marie uniform has been designed to provide comfort, support and freedom of movement, while creating a sense of belonging and equality amongst our students.
Following their first six weeks at San-Marie, students are required to wear the correct uniform for their class. Dance shoes should be correctly fitted, hair must be appropriately tied back and students should follow the uniform requirements for their particular class.
Correct dancewear is also important for safety and allows teachers to see and correct posture, alignment and technique.
Students arriving in clothing, footwear or accessories that are unsuitable or unsafe for their class may be asked to remove the item. Where it would not be safe or appropriate for the student to participate, they may be asked to observe the class instead. Jewellery must be removed where required for the class or where it may present a safety risk
Bags & Personal Belongings
Students should bring a small bag into class containing their dance shoes and water bottle. All personal belongings should be clearly labelled with the student's name.
Any additional belongings should be left in the changing room to help keep the reception and teaching areas safe and free from clutter.
Items left unattended around the building will be moved to the lost property area.
San-Marie cannot guarantee the safe return of lost or unlabelled personal belongings.
Parent/Student Agreement
I confirm that I have read and understood San-Marie Stage School's Uniform & Personal Belongings requirements and agree to follow them.
FEES, PAYMENTS & CANCELLATION
Annual Fees
San-Marie Stage School's fees are calculated according to the number of scheduled lessons for each class during the academic year.
The total annual fee is divided into 12 monthly instalments, payable from August to July. This allows families to spread the cost of the academic year evenly rather than paying larger amounts during teaching months.
The monthly instalment therefore does not represent the number of lessons taking place within that particular calendar month.
Students Joining During the Year
Students joining San-Marie part-way through the academic year will only be charged for their remaining scheduled lessons, following completion of any agreed complimentary trial sessions.
Their remaining fees will be calculated and a payment schedule provided when they enrol.
Invoices & Payment
Invoices are issued through Class Manager and monthly payments are due in advance.
Parents may choose to pay by the payment methods currently offered by San-Marie, including Autopay.
When setting up Autopay for the first time, the first invoice must be paid manually, as Autopay can take approximately one week to become active.
Late Payment
Where payment has not been received, a payment reminder will be issued.
If fees remain unpaid after the 10th of the calendar month, a £5 late-payment charge may be added to the account.
Further late-payment charges may apply where fees continue to remain outstanding.
Students may be unable to attend classes while their account remains in arrears.
Student Absence
Fees remain payable when a student is absent from class, including absence due to illness, holidays or other personal commitments, as their place in the class continues to be reserved for them.
San-Marie is unable to refund or credit fees for individual student absence.
Leaving San-Marie
If a student wishes to leave San-Marie or discontinue a class, one calendar month's written notice is required.
The notice period begins on the date San-Marie receives written notification, and fees remain payable until the end of the notice period.
Refunds
Class fees, examination fees, show fees, costume fees and other payments made to San-Marie are non-refundable once paid, except where San-Marie determines otherwise or where a refund is required by law.
Non-Attendance
If a student is absent for three consecutive weeks without San-Marie being notified, their place may be considered relinquished and offered to another student.
Any outstanding fees will remain payable in accordance with these Terms & Conditions.
Agreement
I confirm that I have read and understood San-Marie Stage School's Fees, Payments & Cancellation Terms and agree to abide by them.
FINAL DECLARATION
I confirm that the information I have provided on this form is accurate to the best of my knowledge.
I understand that it is my responsibility to inform San-Marie Stage School of any relevant changes to the information I have provided, including changes to medical information, allergies, additional needs or emergency contact details.
I confirm that I have read and understood the policies, permissions and Terms & Conditions contained within this form and that I have indicated my choices where individual consent is required.
